The mango is already in the blender. You watched her pick it, split it, and drop it in, and the whole thing has been perfect right up until the moment she reaches under the counter, pulls out a bag of crushed ice, and starts packing the glass. Now you are doing the math every traveler does, quickly and badly, while a line forms behind you and she waits with the scoop in her hand.
Most of us resolve it one of two ways. Either you wave off the ice and feel vaguely virtuous about it, or you take it and then spend two days reading every twinge in your stomach as a verdict. Neither of those is knowledge. They are two different ways of guessing.
The better question is not whether ice is dangerous. It is where the water came from, because that same water has more than one route to you, and the ice is just the one you happen to be watching.
Ice is a proxy. The CDC’s instruction to travelers is to avoid ice unless you are reasonably certain it was made with safe water, which puts the risk exactly where it belongs, on the water supply rather than on ice as a category. Frozen water from a treated municipal system is as safe as the water it came from. Frozen water from a tap you would not drink is that tap water, delayed.
That second sentence is worth sitting with, because most people carry a quiet folk theory that freezing sterilizes something. It does not. The CDC’s hepatitis A guidance states plainly that freezing does not inactivate the virus and that it can be transmitted through ice and frozen foods, which is also why outbreaks have been traced back to frozen berries. Cold slows things down. It does not clean them.
So the useful version of the question, the one you can ask with three words of the local language and some pointing, is where the ice came from. In a lot of countries the ice in a busy stall arrives in sealed bags from a commercial plant that treats its water, and it looks the part: uniform cubes or machine-cut chips, clean bag, kept in a cooler that gets emptied daily. Ice hand-chipped off a block of unknown origin, sitting in melt water since morning, is a different product with a different history. Same glass, different risk, and you can often tell them apart just by looking.
Sealed bottled water is a good default. The CDC calls factory-sealed bottled water the safest option for most travelers, and there is nothing clever about arguing with that.
The problem is that people treat it as a force field. You buy the case, you drink only from it, and you consider water handled. Meanwhile the local tap is still reaching you through the ice in your drink, the herbs and lettuce rinsed at the back of the kitchen, the film of water inside a freshly washed glass, and the mouthful you swallow while brushing your teeth. That is not my list of hypotheticals. It is the exact list the CDC gives when it warns that tap water might be unsafe for drinking, preparing food and beverages, making ice, cooking, and brushing teeth.
Which is why the numbers stay so stubborn. Travelers’ diarrhea attack rates run from 30% to 70% of travelers over a two-week period, depending on destination and season. That is not a range you move much by changing only what you pour into a glass while leaving four other doors open.
There is also a caveat most travelers never hear. The CDC notes that some bottled water is risky depending on where it came from and how it was treated, which is a diplomatic way of describing a real refill trade in some markets. What you are buying is not the brand on the label. It is the intact factory seal. Check it with your thumb before you pay.
If you are somewhere the tap is not potable and you would rather not buy your way through the trip, you have three real options, and each one has a specific blind spot worth knowing.
Boiling is the gold standard and it is not close. Bring water to a rolling boil, and the CDC recommends holding it there for a full minute, which is less about biology than about user variability. The extra time is a margin of safety for the fact that people misjudge when a boil is really a boil. Water that gets there is disinfected, full stop.
Filters are where travelers get overconfident, and it comes down to pore size. Portable microfilters, meaning anything rated under 1 micron, will readily remove bacteria and protozoan parasites including Cryptosporidium and Giardia, which are the two most likely to ruin the back half of your trip. Those same microfilters do not reliably remove enteric viruses such as norovirus, which averages around 0.03 microns and passes straight through. If you want viruses handled by filtration, you need an ultrafilter rated to 0.01 microns, and most of the filters sold to travelers are not that.
Chemical treatment, the tablets and drops, is the most packable option and has the sharpest edge case. Chlorine and iodine based disinfection handles most things well, but Cryptosporidium is poorly inactivated by it at practical concentrations. So the tablets that fit in a shirt pocket are the ones least suited to exactly the parasite most associated with contaminated surface water.
The honest summary is that no single tool covers everything. Somewhere with real municipal treatment and questionable plumbing, a filter is plenty. Somewhere drawing on surface water with poor sanitation, boil it or carry a purifier that deals with viruses.
Here is where good intentions actually die, and it has nothing to do with pathogens.
You boil or filter a liter at breakfast, feeling organized. The bottle then rides in a daypack through a market in 90 degree heat, then in the back of a van with the windows down, and by two in the afternoon it is roughly bath temperature and tastes faintly of the plastic it has been sitting in. You do not want it. So you buy a cold sealed bottle at the next stop, and another after that, and the refill plan you arrived with quietly becomes six bottles a day and a pile of plastic in a country that has nowhere good to put it.
The fix is unglamorous and it is about temperature, not treatment. Water that is still genuinely cold in the afternoon is water you keep drinking, which was the entire point of treating it in the morning. A vacuum insulated bottle such as Polar Camels, are built to hold ice for more than 24 hours. It keeps that liter cold through a full day of transit and heat, so the water you made safe at seven in the morning is still water you want at three in the afternoon. It is a habit problem solved with hardware, which is usually how habit problems get solved.
There is a second benefit that brings us back to the stand. If you are carrying your own cold water, the ice question stops being a coin flip you have to take. You can decline the ice, keep the juice, and still drink something cold.
All of this assumes you are somewhere the tap is a problem, and often you are not. Tap water is safe to drink across most of Western Europe, Japan, Australia, New Zealand, Canada, and plenty of other places, and travelers who buy bottled everywhere out of reflex are spending money and generating waste for no health benefit whatsoever. Check the destination before you go instead of applying one blanket habit to every country you visit. The CDC publishes destination specific guidance for exactly this reason.
And if your itinerary is built around eating in places your hotel concierge would raise an eyebrow at, the highest yield precautions are not about water at all. They are hands and heat. The CDC’s food guidance leads with fully cooked foods served hot, and food you prepare yourself, as the safest category, and it recommends washing hands with soap whenever available, with hand sanitizer of at least 60% alcohol as the backup before you eat. A stall doing serious volume, cooking to order, handing you something still steaming, with a line of locals in front of it, is a better bet than a quiet air conditioned restaurant running a lukewarm buffet. Volume and heat beat ambiance every time.

Ask where the ice came from. If it came out of a sealed commercial bag, or the place is busy enough to be tearing through machine ice all day, take it. If you cannot tell, and you are somewhere the tap is not potable, wave it off and drink the juice warm, which is a very small loss. Either way, drink the juice. You did not fly across an ocean to stand in front of fresh mango and order a sealed bottle of water.
The goal was never to seal yourself off from the place. It was to stay well enough, long enough, to keep tasting it.
